2009 Christmas Hamper Registration & Pick-Up Information
**Please note: There are no appointments to register for the Christmas Hamper and no late registration**

We require identification, proof of address and proof of income for ALL members of the household (including roommates, friends, spouses, and children)

I.D. (Acceptable I.D. – Health Card, Driver’s License, Birth Certificate, Social Insurance Card, Landed Immigrant Papers, OW Drug Card, O.D.S.P, CPP, OSAP,etc.) (Must Be ORIGINAL I.D.– NO PHOTOCOPIES)

Income Statement (From ALL sources) (Must Be ORIGINAL Statement– NO PHOTOCOPIES)

Proof of Address

Individuals Receive: A full chicken (or equivalent to purchase one), 1 10lb bag of potatoes, 1 5lb bag of carrots, 1 box of food

Families receive: A turkey, 1 10lb bag of potatoes, 1 5lb bag of carrots, 1 box of food, 1 bag of toys for children 0-12

**PLEASE NOTE: Registration and Pick-Up will be completed at the same time. Please prepare to take your items with you.**
